In order to facilitate clinical studies of MR guided hyperthermia in the prostate (and pelvis), we plan to devise and implement modifications of controlling systems to the InSightec ExAblate 2100 Prostate System to allow protracted hyperthermia delivery under multi-slice or volumetric MR thermal guidance. Treatment strategies will be developed and assessed using advanced patient specific modeling studies. The modified endorectal ablation system will be evaluated for delivery of hyperthermia in ex vivo tissue and phantom, and perfused tissue in vivo. The results of this study will provide data supporting an IDE application & techniques for applying clinical studies of MR guided hyperthermia within pelvic disease sites. This MR guided approach for administering targeted and protracted (vs. pulsed) hyperthermia to prostate, and possibly other pelvic sites such as cervix and rectum, has potential to significantly enhance efficacy of drug delivery, immunotherapy, and/or hypofractionated radiation therapy
